Protein targets on a GLP-1: the numbers

A common range is 1.2–1.6 g of protein per kilogram of body weight per day, and your clinician sets your own target. Here's what that range means in grams at your weight, and how to eat it when your appetite is gone.

The number

A common range is 1.2 to 1.6 grams of protein per kilogram of body weight a day while you’re on semaglutide or tirzepatide. At 200 lbs, that’s about 110–145 grams a day. It isn’t right for everyone. Your clinician sets your own target, and may change it if you have kidney disease or other nutrition needs.

For comparison, the RDA (the official daily recommendation) is 0.8 g/kg. That’s a floor for adults whose weight is stable, and on a GLP-1 your weight isn’t stable. The 1.2–1.6 range is the standard way to protect muscle mass during rapid weight loss.

This number is borrowed from other research. No one has run a large trial testing different protein targets in GLP-1 users specifically, so the range comes from the broader weight-loss literature. We use it as a starting point because the reasoning is strong, and your clinician adjusts it to fit you.

Your weight, in grams

  • 160 lb (73 kg): 90–115 g/day
  • 180 lb (82 kg): 100–130 g/day
  • 200 lb (91 kg): 110–145 g/day
  • 220 lb (100 kg): 120–160 g/day
  • 250 lb (113 kg): 135–180 g/day

In meals, that looks like three meals built around 30–40 g each, plus one 20–25 g snack, which gets you into the range at most weights. Above 220 lbs, the top of the range takes a second shake.

At higher body weights the per-kilogram math climbs fast, and many clinicians base the target on goal weight instead of current weight. Decide which weight to use with your clinician.

Why the number is high

Semaglutide and tirzepatide cause weight loss one way: you eat less. They slow gastric emptying (how fast food leaves your stomach) and act on the parts of your brain that drive appetite, so your food intake drops sharply and stays down.

The losses are large. In STEP 1 (n=1,961), people on brand-name semaglutide (Wegovy) lost 14.9% of their body weight at 68 weeks, and in SURMOUNT-1 (n=2,539), people on brand-name tirzepatide (Zepbound) lost up to 20.9% at 72 weeks. Weight loss that fast always takes some lean tissue along with the fat. In DXA body-composition scans, a real share of what people lost was lean mass, which includes muscle.

Protein is the half of the fix you can eat. When you’re eating less, your body is constantly breaking muscle down and rebuilding it, and your protein intake decides whether the rebuilding keeps pace. The other half is giving your muscles a reason to stay, which means strength training about twice a week, covered in strength and recovery.

What we don’t know is whether the lean mass lost on these drugs will show up as weakness or lost function five or ten years from now. That data doesn’t exist yet, and our position is to protect the muscle anyway. If long-term studies show strength holds up without the extra protein, we can relax this advice. Until then, it’s a sensible step for most people, with your clinician’s OK.

Eating it when you’re not hungry

Knowing the number is easy. Eating 130 g on a stomach that fills fast and empties slowly is the hard part. Three rules do most of the work:

  • Protein first, everything else second. Fullness arrives early and ends the meal, so if the chicken comes after the rice, the chicken doesn’t get eaten.
  • Don’t save protein for dinner. If you’re 80 g behind at 6 p.m., you won’t catch up. Get 30 g in before noon.
  • Let liquids carry the bad days. A 30 g shake, Greek yogurt, or cottage cheese will go down when nothing else will, and they count just as much as chicken does.

For meal ideas, see twelve meals that hit 30 g in under 10 minutes. The GLP-1 food guide covers how protein fits with fiber, fluids, and eating on dose day.

Count for three days

Find your weight in the list above, or use the target your clinician gave you. For three days, log everything you eat that has protein in it, using an app or a notes file, and then measure the gap. Close it one meal at a time, starting with breakfast, because a shake and one better morning meal will do more than any dinner makeover. And if you’re not on a GLP-1 yet and want to know whether you’d qualify, take the quiz.
