To get a prescription, you need an online visit reviewed by a U.S.-licensed provider. Compounded medicines are made by licensed U.S. pharmacies and are not FDA-approved. Results vary from person to person.
Who it’s for
- BMI of 27+ with a weight-related health problem, or 30+ without one
- Tried diet and exercise, but the results didn't last
- Want less food noise (constant thoughts about food)
Most people notice they're less hungry before the scale moves.
How it works
Semaglutide is a once-weekly GLP-1 medicine. It slows how fast your stomach empties, helps your body use insulin better, and makes you less hungry.

What we can and can't tell you
Results are different for everyone. The weight loss only lasts while you stay on treatment. Most people gain weight back after stopping if they don't have a plan to keep it off.
- Who makes it A LegitScript-verified 503A compounding pharmacy makes it from your prescription. We never use gray-market or overseas suppliers.
- To what standard USP <797> standards for sterile compounding, and USP <800> standards for handling hazardous drugs, where they apply
Compounded medicines are not FDA-approved. The FDA does not review them for safety or effectiveness. They are not generic versions of any brand-name drug, and they are not equivalent to one.
What the first months look like
Starter dose (0.25 mg). You'll feel less hungry first. The scale stays put for now, and that's expected.
Your dose goes up about every 4 weeks. Side effects tend to show up after each step. Then they fade as your gut gets used to it.
Maintenance dose (the dose you stay on). This is when the real change builds. It happens over months, well past the first few weeks.
Check-ins, small dose changes, and a real plan before you ever stop. If weight comes back after stopping, that's your body's biology. It doesn't mean you failed.
Dosing, step by step
| Start | 0.25 mg weekly — a low dose so your stomach can adjust; it isn't for weight loss yet |
|---|---|
| Step 2 | 0.5 mg weekly — ~week 5 |
| Step 3 | 1.0 mg weekly — ~week 9 |
| Step 4 | 1.7 mg weekly — ~week 13 |
| Target | 2.4 mg weekly — most members stay between 1.0–2.4 mg |
Your clinician sets the pace. Staying on a step longer because of side effects is normal.
And when it gets rough
Show what helps
- Nausea Eat smaller meals, eat slowly, and go easy on fat. Nausea tends to follow dose steps, then fades. If you keep throwing up, contact your clinician the same day.
- Constipation Drink more fluids first. Then add fiber, then magnesium. Check with your clinician if you need more help.
- Fatigue (early) Usually it's about what you take in. Drink enough water, get your electrolytes (salts like sodium and potassium), and eat your daily minimum of protein.
- Reflux Eat a smaller dinner, earlier. Raise the head of your bed. Tell your clinician if reflux is new or getting worse.
Safety information
A U.S.-licensed clinician reviews every prescription and decides whether this medicine is right for you. During your visit, tell them if any of the following applies to you or your family.
In rodent studies, semaglutide and tirzepatide caused thyroid C-cell tumors. It is not known whether they cause these tumors, including medullary thyroid carcinoma (MTC), in people. Do not use this medication if you or any of your family have had MTC, or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
Full GLP-1 safety information → See if I qualifyBefore choosing a weight plan, we go over your health history, your goals, and your options. Compounded medicines are not FDA-approved. The FDA hasn't reviewed them for safety, effectiveness, or quality. Results from trials of brand-name drugs don't prove the same results for a compounded version. Brand-name options are FDA-approved, and you can ask us how they compare for you.
