Risk of thyroid C-cell tumors. In rodent studies, semaglutide and tirzepatide caused thyroid C-cell tumors. It is not known whether they cause these tumors, including medullary thyroid carcinoma (MTC), in people. Do not use this medication if you or any of your family have had MTC, or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
There is no dose you have to reach.
We start low. Some people stay low and never climb. That’s a plan, not a failure.
The lowest dose that works is the goal
Everyone starts here. Week‑one nausea is common and fades.
Only if you need it
You move up one step only if you’re still hungry. Nothing happens on its own.
Where most people notice a change
Your appetite is smaller. Many people stay at this dose.
Still your call
Some people go higher. Some don’t need to. On a compounded plan, the price is the same either way.
The dose that works for you
You stay on it as long as it works. Check‑ins keep you there, or lower it.
How GLP‑1 medicines work.
GLP‑1s act on the parts of your brain that control hunger and fullness.
They slow down how fast food leaves your stomach, so a meal keeps you full longer.
They help your body release insulin after you eat. Tirzepatide also works on a second hormone, called GIP.
Compounded medications are not FDA‑approved. They are not a generic version or equivalent of any brand‑name drug. Individual results vary. Safety information →

Read on Trustpilot (review by Beth)“At 51 years old, I honestly didn’t think I could turn back the clock the way I have — but this program has helped me in every way possible. I’ve lost a sizable amount of weight but It’s not just about the weight. It’s my energy, my confidence, my discipline, and my mindset. My joints don’t hurt like they used to. I sleep better. I move easier. I feel lighter physically and mentally. And you truly can’t put a price on your health. Their customer service is amazing — I’ve literally received responses within minutes. Any time I’ve had a question or concern, it’s been handled quickly and professionally. This wasn’t overnight. It was consistency. And Zappy gave me the structure to stay consistent. Common questions.
Do I need insurance to start?
No. Zappy is cash-pay. Most insurance plans don't cover weight-loss medicine, and regular Medicare can't cover it by law, apart from a temporary program for some brand-name medicines. You can use FSA/HSA cards for eligible costs.
Is compounded semaglutide the same as Wegovy?
No. Wegovy is the FDA-approved brand. Compounded semaglutide is made by LegitScript-verified 503A pharmacies, with a doctor overseeing the process. It is not FDA-approved. It is not an FDA-approved generic or equivalent of Wegovy.
How fast will I see results?
Many people feel less hungry within 1–2 weeks. Weight usually starts to change around weeks 4–8, as your dose goes up slowly and steadily.
What if I have side effects?
Tell your clinician. They can slow down how fast your dose goes up. Nausea, tiredness, and constipation usually get better as your body gets used to the medicine. You can message your care team anytime.
